Getting Back on Track: How to Reach Pediatric Patients for Preventive Needs Using Population Health Tools and Mobile Van Service to Aid in This Effort

ABOUT
This training will address community outreach required to engage patients and families with immunizations and other preventive services that meet their needs and address access barriers for hard-to-reach pediatric populations. It will share how to effectively use population health tools for specific mobile health initiatives and demonstrate how hard-to-reach populations can benefit from mobile Medicaid enrollment, particularly with the COVID-19 Public Health Emergency (PHE) declaration ending. Speakers will also cover how the PHE ending impacts engagement for patients and professionals. The training will also provide an understanding of the National Committee for Quality for Quality Assurance based recommendations on the effectiveness and use of mobile health programs.

Dr. Avian Tisdale has over a decade of progressive leadership, serving as Chief Medical Officer, Division Director, Clinical Assistant Professor, and Hospitalist for complex multi-site systems. In these varied roles, she cultivated strong partnerships with external stakeholders, including Nemours DuPont, Jefferson University Hospital, and Rowan University School of Osteopathic Medicine.
She has successfully built reliable and translatable enterprise-wide quality programs for value-based practice models. She also has direct experience building a successful Telehealth program in a multi-site hospital system. As Chief Medical Officer, she chaired committees for quality, credentialing, and population health. She specializes in pediatric quality improvement.

Josh Glueck

Josh Glueck began his career as a consultant directly involved in the transfer of a hospital-owned health clinic to the community-based organization, Berks Community Health Center, located in Reading, Pennsylvania. With Josh’s team, this organization became a Federally Qualified Health Center (FQHC) through the competitive grant application process. Josh’s core roles consisted of leading strategic planning to ensure a successful transfer, networking with local hospitals and community organizations, establishing and directing the Quality Improvement/Assurance Committee, grant writing, establishing contracts with Medicaid managed care organizations, contracting and credentialing physicians with various insurances, managing contract agreements and organizational policies, creating a risk management plan, and many other duties.
Josh stayed with Berks Community Health Center for several years serving a quality improvement and operations role within the organization. In 2014 his organization began to use i2iTracks and since Josh has an affinity for technology and data, he applied to i2i Population Health. He joined the team in April, 2015 and served as a Regional Director/Population Health Consultant (PHC) based out of Philadelphia, PA until May, 2017 when he was promoted to the role of Director of Training and Education. In this role, Josh leads the team of Population Health Consultants training both internally and externally and he works directly with the Product Team in guiding future enhancements of i2i solutions.
